Insurance claims will be submitted on the day of service. If they do not cover
100%, the patient will be billed for the difference. Payment is due upon
receipt of the billing statement.
Dental Treatment:
The patient is responsible on the day of service for endodontic treatment. If
treatment requires more than one visit, payments may be spaced out until
treatment is finished.
Patients with insurance:
Insurance will be submitted on the day of service, Insurance will not cover
100% of any endodontic treatment. We estimate that they will cover between
50-80% of treatment, depending on the procedure done. We ask for the patients
co-pay at the time of service. Estimates are an approximation of insurance
payments. If there is still a balance once insurance is received then patient
will be billed for the difference. Payment of the bill is due upon receipt. In
the event that the insurance carrier should pay more then we estimated, we
would be more then happy to give the patient whatever refund is due them.
For your convenience Visa, MasterCard, American Express and personal checks are
accepted forms of payment.
